Please feel free to reach out anytime with questions or comments about how we can best serve your student.

Information:

  • Oregon law requires that all students attending school must be up to date with their immunizations or have an appropriate medical or non-medical exemption in place. Every February, the state determines a day that students must be excluded from school if they do not have up-to-date records.

*Information about what immunizations are required can be found here on the Oregon Health Authority website (multiple languages are available).

*Information about how to obtain a non-medical exemption can be found here (multiple languages available).

    *Places to receive vaccines:

-Make an appointment with your primary care provider

-Call Clatsop County Public Health at 503-325-8500

-Many local pharmacies offer vaccines, call them directly for availability and appointments.

-Call 211 or email help@211info.org to locate other providers, multiple languages are available.

  • When students are ill, it’s best to keep them home until they are well. This helps ensure they get the rest they need to quickly recover and it helps prevent the spread of illness at school. Please click on these links for guidelines on when to keep your students home. English   Spanish

  •

Oregon law heavily regulates how medications are dispensed at schools. Please read this document (English and Spanish) to understand the policies and procedures before you bring any medication to school.

All medications must be medically necessary for your student to remain at school.

Please remember that a parent or guardian must transport and check in all medications to the school.

Please do not send any medications, including over-the-counter medications and cough drops, with your student.

The required medication authorization form that parents must fill out can be found here (English and Spanish).

Students that are developmentally and behaviorally able may be allowed to self-carry and self-medicate some medications. Parent authorization and Seaside School District Administration authorization is required. Additional forms/information may be required depending on the type of medication that is being self-administered. The self-medication contract can be found here (English and Spanish).

  • All students with life-threatening allergies, asthma, seizures, diabetes, adrenal insufficiency, and other health conditions that require emergency medications, emergency response or daily nursing care (including but not limited to G-tube feedings and catheterizations) will require a nursing health plan. If your student has any of the above-listed conditions or needs planned nursing care and support at school, please contact Nurse Dani. All health plans are renewed each school year and may require new doctor’s orders each school year. Please note that all orders and prescriptions must be from an Oregon-licensed provider.

  • Opioid overdose reversal medications, also called naloxone or Narcan, can very quickly restore normal breathing for a person whose breathing has slowed down or stopped because of an overdose of prescription opioids or illicit opioids, which includes illicitly manufactured fentanyl and heroin.  The Seaside School District has Narcan available at all of our schools so staff can quickly respond to an opioid overdose situation. Please click here (English and Spanish) for a parent letter and links to additional resources for families.
